Multiple Sclerosis - APEX NEURO Real-World Rehab - Part 1 After foundational work in the clinic, Luke and Gaynor make such great progress that they are ready for some real-world rehab. Our clinic places a lot of value in real-world rehab. As much as walking in the controlled clinic environment can be a huge achievement, we want clients to transfer these skills into everyday life, which is full of challenges.

Real-world rehab allows us to practice and build confidence in uneven surfaces, hills, stairs, distractions and being around other people. In Part 1 of this video, Gaynor is tackling slopes, stairs and uneven surfaces. The temptation to speed up or revert back is understandable - Gaynor has developed a method of walking that is most efficient since her MS progressed, but it’s not utilising the muscles correctly and her foot drop makes her at risk of falling. Instead of swinging the leg or walking toe down which are patterns she’s fallen into over time to help her get from A to B, Luke is teaching Gaynor to lift her foot to engage all her muscles, which over time will improve her speed, safety and confidence. Sometimes the simple solutions can be the best ones - if things start to go wrong, stop. It’s perfectly fine to reset, gather yourself and go again.

Doing this out in the real world is great practice, sometimes there will be people around making you feel rushed - but you have to find confidence in allowing yourself a moment to reset. As you can see, during these breaks, Luke is constantly coaching Gaynor - there is a lot to remember, so Luke is on-hand to help Gaynor with each detail, providing demonstrations so she can see what she needs to replicate. A change in surfaces, a pretty steep embankment and a 30-step set of stairs.

These are no easy feats, but Gaynor takes the feedback and applies it at each section. Keep an eye out for Part 2, as new challenges present themselves that Luke and Gaynor successfully navigate!
